[edit] Health Care for Homeless Veterans HCHV
Health Care for Homeless Veterans (HCHV) is considered the entry point and first stop for veterans who are homeless. HCHV provides a wide range of social and treatment services, with the goal of providing stabilization and linkage to needed services. Intakes are done daily. At intake the veteran is assigned a case manager who they develop short-term goals with. Services provided include emergency shelter referrals, placement in transitional housing, referrals to substance abuse treatment, medical triage, psychiatric evaluations and medication follow-up, and basic services such as lockers, telephone use, and transportation for appointments, access to showers and laundry. HCHV also provides a number of drop-in groups and classes on daily basis. HCHV has three social workers available on a drop-in basis in the morning. Medical Services, Mental Health Services are also located at the SF VAMC Downtown Clinic with HCHV.

To make an appointment to obtain a primary care physician, patients or patient's representatives may call the medical line, 551-7376. They may also get to that extension by calling the main number 551-7300 and pressing option 1 for medical.
Patients may also walk in to make appointments. If they come earlier in the day (9am), there is a greater likelihood they can be seen the same day. When the clerks make appointments, they also check eligibility if the veteran is not in the VA system. We have new patient appointment access within 7 days, usually within a few days.
[edit] Compensated Work Therapy/Transitional Residence (CWT/TR)
Designed to help veterans make the transition from homelessness to independent employment and community living. The program provides case management and vocational experience in a workshop or transitional work placement. Veterans are required to be clean and sober and should be referred by their case manager or outpatient treatment provider.Call 415 551-7323
