On Lok Senior Health is a health plan especially designed to cover acute and long-term care for older adults, featuring full medical care, prescription drugs, home care, adult day health and transportation. Eligibility Requirements: 55 years or older and living in San Francisco; experiencing ongoing medical problems, memory loss, and/or need daily help with bathing, walking, eating, or dressing; and, may be considering a nursing home but prefer to stay at home.
Wheelchair accessible. Most staff are bilingual in Chinese or Spanish. On Lok serves many ethnic groups, including Chinese, Vietnamese, Filipino, Korean, Central and South American, Mexican, African-American, Italian and Russian.
On Lok is not a nursing home.
