Provides eye exams and prescriptions for eyeglasses, does not provide eyeglasses. Provides funds for eyeglasses in specific circumstances. Ophthalmological services including eye surgery, and glaucoma/cataracts assistance. Accepts referrals for free eyeglasses for homeless patients after approval by the Social Services Department. A letter from agency verifying homeless status should be sent with patient to Social Services Department 1C6. Social Services staff will evaluate and determine eligibility for payment of eyeglasses. If eyeglasses are approved by Social Services Department, a voucher(s) will be issued to patient(s) from Department 1C6. (Note: Patients are then sent by Social Services staff to a designated source for their eyeglasses). Usually 1/2 to one month wait. Fees on sliding scale. Spanish, Cantonese, Tagalog, Vietnamese, Russian. Wheelchair accessible.
