Eligibility for S+C requires clients to be chronically homeless, living on the streets or in a SF emergency shelter and have a diagnosed disability related to the one or more of the following:
1) Substance (ab)use
2) Serious mental health issues
3) Disabling HIV/AIDS
S+C provides rental assistance for clients occupying units in buildings owned and operated by various housing providers. S+C services include, but are not limited to, onsite case management, basic living skills training, benefits advocacy, support groups, mental health services, crisis intervention and vocational training.
